My Online Clinic allows our office to communicate with you securely. The best way to create an account is to respond to the email we send you after an appointment which contains a link that takes you to the patient portal. You can also visit our website at www.HoustonEyeDoctorVital.com and click the My Online Clinic button:

 You will then be taken to a LOGIN screen where you can Create Your Account.

What is a patient portal account?

A patient portal account is an account you create online to communicate securely with us. This will require that you create a user name and password and that you provide some additional information about yourself such as your name, phone number, date of birth and email. Your patient portal allows you to securely:

  • View chart information from your most recent appointment.
  • Receive patient education after your visit.
  • Exchange secure messages with our office including requests for refills on your medications.
  • In the future we hope to add the ability for you to schedule appointments online and make secure payments.

How do I create my patient portal account?

The best way to create a patient portal account is to respond to emails we send you after an appointment. This email will invite you to click on a link which will take you to the patient portal. From here you will be asked to confirm your identity and then to create a portal account if you don’t already have one. Alternatively, you may create a patient portal account on your own from the practice’s website at any time.

Why create my portal account directly from the practice’s website?

Creating an account independently from receiving a token in email from us means we may need to further confirm your identity after you have created your patient portal account and before you can view your medical record. You may need to contact us if you do not see your medical information after creating a portal account. When you create your portal account from an email we send you, your medical record is automatically tied to the new portal account you create.

Why do I need to create a portal account?

We take great care to ensure your health information is kept private and secure. By having an account, you have 24/7 access to your patient portal! You are able to view your chart, send messages to your HEA doctors and access other features the clinic has to offer. The login approach is a secure way to see your private information.

What if I forget my username and/or password?

We know this can be frustrating, so we’ve made it easy to recover.
To recover your username: From the login page of the portal, click on ‘forgot’ and it will prompt you for your email. Your username will be emailed to you to get back in the system.
To recover your password: From the login page of the portal, click on ‘forgot’ and it will prompt you with your username and the answer to your secret question. Your temporary password will be emailed to you to get back in the system. It’s important to change your password back to something permanent and more memorable.

Why am I not getting my reset email?

Your new temporary password was emailed to the email address we have on file for your account. Make sure you check that email address and check your spam filters in case this email was incorrectly identified as junk email.

I forgot my username and have input my email, but now I am getting an error. What do I need to do?

In order for us to email you your username, we need to be able to locate your email address and it needs to uniquely identify your one username. It may be that you provided a different email address when you created the account. It may also be that a family member or someone else provided the same email address when they created their own portal account. The result is that we cannot identify the correct one unique username to send you. Please contact us so that we can look up your username for you.

What if I don’t know the answer to my secret question?

No problem. Please contact the practice and we will retrieve this information for you.

What are the requirements for creating a username and password?

To create a username: You must include at least 8 characters and may include numbers.
To create a password: Include at least 8 characters, 2 numbers, and no special characters.

This is my first time trying and why can’t I see my chart when I open my email or after logging in to the portal?

This will happen if your medical record has not been linked to your portal account. You will need to call us or send a secure message asking us to link your account. To send a secure message, log into the portal and create a new message asking us to link your account. Once this has been completed, you will be able to see your chart information.
